* Two letters of recommendation from individuals who can attest to the quality of your academic performance and, if applicable, of any research experience you might have (requested of candidates applying to a Ph.D. program only). Please note that letters of recommendation must be confidential and therefore sent directly by the persons who are recommending you.
* An official transcript of your academic record.
* Your resume which should include all awards and honours you have received.
